Who is Muslim and Momin?
Muslim
A Muslim is someone who submits to Almighty Allah's will. A person upon true monotheism, who worships Almighty Allah alone without associating any partners with him. A Muslim is someone who Bears witness that None has the right to be worshipped but Allah, and Muhammad is the Messenger of Almighty Allah. A Muslim can be of any background, race, country and gender.

The Declaration of Faith in Islam. There is no Deity worthy of Worship except Allah and Muhammad is his Messenger

Muslim is one Who Accept believe Islam as the true religion and not to reject any of it's necessary aspects.
 to accept each and every word of Sayyiduna Rasoolullah (sallal laahu alaihi wasallam) as the absolute truth and to testify whole-heartedly the reality and truth of Sayyiduna Rasoolullah (sallal laahu alaihi wasallam).
 Any of his words, actions or conditions does not reject, insult or falsify Allah Ta’ala and His Rasool (sallal laahu alaihi wasallam).
 perform actions that are signs of Kufr For example, to wear a "Janeo" (a sacred thread worn by Hindus), or to have a lock of hair (plats for men) the same as Buddhists have, or to wear a cross are all actions of Kufr.
How to Become a Muslim
1  Have a good mindset of Islam. Rest assured that the path to Islam is indeed very simple. Understand that in Islam one doesn't convert rather one reverts back to an "original" natural belief.In Islamic understanding every child is born innocent and stays so until puberty, thus all children that die young are taken directly to heaven. So when one "converts" to Islam he/she is actually reverting to their own original nature:

2 Testify your faith by reciting:

أشهد أن لا إله إلا الله وأشهد أن محمد رسول الله
Ash-hadu alla ilaha illallah-wa ash-hadu anna Muhammadan abduhu wa rasulullah"
I bear witness that there is no God except Allah and Muhammad (SAW) is His messenger".
By reciting this statement (and believing in it), you become a Muslim.
3 Purify yourself:
 Purify yourself  before or after Testifying your faith Immediately upon becoming a Muslim, you should take a shower, to purify yourself. This is both a symbolic act of washing away the past and being purified, of emerging from darkness to light, created afresh and also signifies the importance of purity in Islam
Once you have entered into Islam and made sincere repentance to Allah, you should consider your past minor sins forgiven and strive to improve your spiritual state by doing good deeds.

4 Believe in the seven articles of Faith:
     * Believe in Allah Almighty,
     * His angels
     * His revealed books
     * His messengers (from Adam to Hazarath Mohammad(sallalahu alihi wasalam)
     * The Last Day The day of judgement
     * the fate (good and bad )predestined by    Allah Almighty
      * and the Resurrection after Death.
 This forms part of the 1st Pillar

If you do accept the above all mention in his life long and die with the same belief surely He/She will go to heaven.

Momin
 To be a muslim, one has to do above mention points of muslim,but to be a momin one has to follow the religion of Islam completely not only in words but also in spirits.
